In de verschillende WMG-overeenkomsten zijn de volgende beroepen aangemerkt als bevoegd hoofdbehandelaar:
 
''Ervaren probleem''<br/>De regeling ‘declaratiebepalingen DBC's in de curatieve GGZ’ schrijft sinds 2013 voor wie hoofdbehandelaar mag zijn. Over 2012 en eerder waren er nog geen publieke eisen. Over 2013 geldt als eis dat de hoofdbehandelaar een beroep moet hebben dat opgenomen is in het BIG-register, en bevoegd en bekwaam moet zijn om cliënten te classificeren volgens de systematiek van de DSM-IV-TR. Ook moet de hoofdbehandelaar vanaf 2013 direct cliëntgebonden tijd hebben besteed aan de cliënt waarvoor wordt gedeclareerd. Over 2014 is het beroep van de hoofdbehandelaar verscherpt naar een gelimiteerde set van elf beroepen, waarvan twee specifiek voor dyslexiezorg. Verzekeraars hebben deze regels geoperationaliseerd in de modelovereenkomst en in de contracten met zorgaanbieders. In de praktijk zijn situaties ontstaan waarin het beroep van de hoofdbehandelaar niet aansluit bij de gemaakte contractafspraken. De vraag is of de verzekeraar deze behandelingen wel mag vergoeden.
 
''Uitwerking NZa''<br/>Als zorgaanbieders en zorgverzekeraars ervoor kiezen om over 2013 nadere afspraken te maken die passen binnen de regels over wie hoofdbehandelaar mag zijn, dan is dat aan partijen. De NZa gaat niet over private afspraken tussen aanbieder en verzekeraar, maar houdt wel toezicht op naleving van wet- en regelgeving. Dit houdt concreet in dat de NZa vanuit haar toezichthoudende rol geen belemmering ziet als een verzekeraar een hoofdbehandelaar accepteert waarvan het beroep niet aansluit bij de eerder gemaakte contractafspraken. Voorwaarde is dat dit beroep aansluit bij de regeling ‘declaratiebepalingen DBC’s in de curatieve GGZ’ en dat de verzekeraar deze aanpassing publiceert.
