Mogelijkerwijs zijn er door ggz-instellingen met specifieke verzekeraars afspraken gemaakt die afwijken van hetgeen hierboven genoemd is. Het is bijvoorbeeld mogelijk dat verzekeraars bepaalde beroepen in het geheel niet toestaan als hoofdbehandelaar. Indien er sprake is van afspraken met verzekeraars die strenger zijn dan de beleidsregels van de NZa wordt dat in de instellingsspecifieke bijlage bij het normenkader nader toegelicht, en wordt daar in de programmeerbare norm rekening mee gehouden.

Toelichting op data-analyse

Lever de lijst aan waarin de hoofdbehandelaars per UZOVI-code zijn opgenomen aan de hand waarvan de controle is uitgevoerd.  

Toetsingskader

  • Indien zorgtype initieel (1xx): Initiële DBC is ondoelmatig (privaat onjuist) wanneer binnen de DBC geen directe tijd door een bevoegde hoofdbehandelaar is geschreven conform de private afspraken met de individuele zorgverzekeraars (Inkoopvoorwaarden & addenda).  
  • Indien zorgtype crisis met verblijf (302): Crisis-DBC met verblijf is onrechtmatig wanneer directe tijd door een bevoegde hoofdbehandelaar ontbreekt.

Er is sprake van  “DBC zonder directe tijd hoofdbehandelaar per verzekeraar (N1002)” als aan de volgende selectie is voldaan: 


1)  Alle initiële DBC's geopend in het betreffende jaar 


2) DBC zorgtype ongelijk aan 301: crisis zonder opname


Primaire diagnose DBC is geen Leesstoornis (diagnosecode as1_1.01.01)



3) Initiële DBC (zorgtype 1xx) is onrechtmatig wanneer er geen directe tijd geregistreerd is door een bevoegd hoofdbehandelaar (conform verzekeraarsafspraken)

3) Crisis-DBC (zorgtype 302) met verblijf is onrechtmatig wanneer directe tijd door een bevoegde hoofdbehandelaar ontbreekt

Berekening financiële impact

De financiële impact volgend uit het zelfonderzoek betreffende doelmatigheid kan voorafgaand aan het zelfonderzoek niet ingeschat worden. Indien inhoudelijk geen overeenstemming wordt gevonden, dan kan een zorgverzekeraar in bilateraal overleg binnen privaatrechtelijke afspraken tot een financiële impact komen.
